Findings

  • Telescoping of the terminal ileum into the cecum and ascending colon extending to the hepatic flexure in the right upper quadrant
  • Small volume free fluid layering in the pelvis and left lower quadrant


Diagnosis

Ileocolic intussusception

Findings consistent with ileocolic intussusception extending to the hepatic flexure.

Discussion

  • Make sure you are familiar with the “telescoped” appearance of an intussusception on ultrasound
  • Notice that on this case, the sonographer applied color Doppler to the intussusceptum to show preserved vascularity


Images

Preserved flow on color Dopper analysis within the intussusceptum (red arrows). Although this is a reassuring finding, the patient is still at risk for bowel ischemia if the intussusception is not reduced.
