Findings

  • Echogenic material in the bilateral lateral ventricles with extension from the left caudothalamic groove into the adjacent brain parenchyma
  • Enlargement of the lateral ventricles
  • Premature sulcation pattern, in keeping with the patient’s gestational age
  • No extraaxial collections identified


Diagnosis

Grade 3/4 germinal matrix hemorrhage

Sample Report

Bilateral intraventricular hemorrhage with extension from the left caudothalamic groove into the adjacent brain parenchyma and corresponding hydrocephalus, consistent with grade 4 hemorrhage on the left and grade 3 hemorrhage on the right.


Discussion

  • Make sure you are familiar with the grading system for germinal matrix hemorrhage
  • The parenchymal component in this case is not particularly well defined and has only mild associated mass effect, which may represent a hemorrhagic venous infarct
  • Periventricular leukomalacia can certainly accompany these findings, so it is important to pay attention on followup imaging for progressive periventricular echogenicity and cystic changes
