Sample Preliminary Report

Right lower quadrant intussusception, favored ileocecal, measuring approximately 10 cm in length with associated small bowel obstruction. Prominent soft tissue within the intussusception is concerning for a neoplastic lead point. Multiple additional areas of nodular small bowel wall thickening and enlarged retroperitoneal, periportal, and inguinal lymph nodes are concerning for neoplastic disease, particularly lymphoma. A nodal conglomerate at the portosplenic confluence exerts mass effect on the main portal vein, which remains patent.

Submucosal edema involving the intussusception, likely relating to venous congestion, though developing ischemia is not excluded. No convincing pneumatosis or portal venous gas. No evidence of perforation.

A peripherally enhancing fluid collection is seen on the right medial thigh suggestive of abscess with likely fistula to the rectum. This can be seen in inflammatory bowel disease.

Findings concerning for a perianal abscess within the right ischioanal fossa which tracts superiorly to the rectum with mild rectal wall thickening and stranding within the perirectal fat. This could relate to underlying inflammatory bowel disease, namely crohns disease.
